To honor everyone’s reserved time, 48 hours noticed is requested for cancellations or changes. Appointments with less notice may be subject to a fee.

Your medical and personal information will not be released to anyone without your prior written consent, except as required by law. This includes other medical personnel, your relatives, and/or friends.
